چکیده
With the development of X-ray technology and advent computed tomography, it became possible not only to accurately diagnose hiatal hernia, but also conduct morphometry all its constituent anatomical structures in different projections use data obtained at stage preoperative planning for surgical treatment. Currently, due improvement quality diagnostics, there is an increase detection patients with this pathology, along this, question choosing most rational method treatment acute. The aim study was tomographic anatomy cardioesophageal junction hernia. A retrospective a series tomograms 53 performed on 64-slice Canon Aquilion Prime tomograph, carried out. An assessment made diameter distal esophagus, size hernia orifice, deviation axis esophagus level hernial orifice relative diaphragm two planes. As result analysis, found that 79% cases located right behind sac, 13% - middle, 8% left behind. It revealed unchanged part above sac 20.9±3.71 mm (min – 15.7 mm, max 30.2 mm). dimensions varied within 31.21±5.23 frontal plane 24.7 42 mm) 32.66±4.36 sagittal 26.2 39.1 63.53°±16.74 19.6°, 92.3°). 74.12°±21.31 36.3°, 118.1°). Thus, variability structure location zone necessary take into account indicators, including presence or absence large branches greater omentum body type, gender, age patients, which serves integral treatment, optimization reduction complications, as well determining safest surgery.

Barium esophagograms of 89 patients out of 110 with manometrically proven achalasia were reviewed. Only four (4.4%) patients showed association of hiatal hernia, indicative of its rare occurrence. Presence of hiatal hernia, is said to have less likelihood of associated achalasia but this association does exist as per our report and that of others.
